Services
Home health services are designed to support patients who are homebound or who prefer to receive care in a familiar and comfortable environment. The goal is to promote healing, improve quality of life, prevent unnecessary hospitalizations, and support safe recovery at home.

Skilled Nursing
Skilled nursing services are provided by licensed Registered Nurses (RNs) and Licensed Vocational/Practical Nurses (LVNs/LPNs) under a physician’s order. These services are designed to meet complex medical needs and support recovery, disease management, and overall health in the home setting.

Physical Therapy
Physical therapy services focus on restoring strength, mobility, balance, and functional independence following illness, injury, or surgery. Licensed physical therapists develop individualized treatment plans to reduce pain, improve movement, prevent falls, and help patients safely return to daily activities within the comfort of their homes.

Occupational Therapy
Occupational therapy helps individuals regain the skills needed for everyday living. Therapists work with patients to improve the ability to perform activities such as dressing, bathing, meal preparation, and home management. Services also include adaptive techniques, energy conservation strategies, and recommendations for assistive devices to promote independence and safety.

Speech-Language Pathology
Speech-language pathology services address communication, cognitive, and swallowing disorders. Licensed speech-language pathologists assist patients with speech and language difficulties, memory and problem-solving challenges, and safe swallowing techniques to reduce the risk of aspiration and improve overall quality of life.

Medical Social Work
Medical social work services provide emotional support, counseling, and resource coordination for patients and families. Social workers assist with coping strategies, care planning, community resources, advance directives, and coordination of services to ensure continuity of care and support during illness or recovery.

Home Health Aide Services
Home health aides provide hands-on personal care under the supervision of licensed professionals. Services include assistance with bathing, grooming, dressing, toileting, mobility, and basic daily activities, supporting patient comfort, dignity, and safety in the home.
